Multimedia Consent and Release Form

I, the undersigned, hereby grant permission to the Recovery Coordination Program (RCP) - including its successors - to use multimedia of me and/or my family, such as quotes, photographs, or videos provided to or taken by RCP.  I understand that RCP may incorporate such multimedia into its communications products, such as a website, printed publications, and presentations, without further consultation or approval from me.  By providing RCP this multimedia consent and release, I agree to waive any copyright claims that I have to this material.

I acknowledge RCP’s right to crop or otherwise prepare these multimedia for publication in any manner deemed necessary.  I also acknowledge that RCP may choose not to use my multimedia or may do so at its own discretion at a later date.  I understand and agree that RCP may use these multimedia in any manner it deems appropriate in furtherance of its stated mission.

I agree to hold RCP and its personnel harmless from any and all claims, liabilities, and actions arising from RCP’s use of the multimedia.  I hereby waive the right to sue and/or lodge a complaint in any forum over the use of these multimedia or those of my family in any publication related to RCP.

I affirm that completing and submitting this form electronically constitutes my signature, which has the same legal validity and enforceability as a manually executed signature.
